I think being pragmatic is only a part of the story. Sure, more people; fewer doctors--you have to be pragmatic. Again, threat of flu; only so many shots--again, you have to be pragmatic. However, I think there's a deeper change taking place, or that has taken place; a different way of thinking. It's first of all an objectivization of people and their problems. And I think it's driven by a growing reliance on "solutions" that are applicable without too much thought. Maybe it's a sort of computer model: a problem arises, and there are things you do. The parallel is with how information and knowledge have come to be identified.

It sure is a deep subject and we'll not settle it here.
.
In a past career I was an advertising account manager. We had certain formulas we could apply to any situation. A set of questions to be answered in a fact finding stage that required research, etc. It wasn't a quick process. But, some 'space cadets' by-passed the formula and went straight to the presentation. Seems like that applies to what the docs are doing today. They see so much of it that they work as though the problem is a problem of the population rather than the specific patient in front of them.

So, what does that portend for medicine in these postmodern times?
